ELD Malfunctions

Paper Logs

If a driver has an ELD malfunction that malfunction must be noted on the drivers logs at the time of malfunction. All drivers are required to have the last 8 days of logs.  All drivers' MUST know how to correctly fill out a paper log in the event that their ELD malfunctions or stops working. 

 The below metrics must be filled out to be deemed legal:


  1. 1. Date
  2. 2. Total daily miles driving
  3. 3. Truck or tractor & trailer number
  4. 4. Name of carrier
  5. 5. Driver's signature/certification
  6. 6. 24-hour period starting time (e.g. midnight, 9:00am, noon, 3:00pm)
  7. 7. Main office address
  8. 8. Remarks
  9. 9. Name of co-driver
  10. 10. Total hours (right right edge of grid)
  11. 11. Shipping document number(s), or name of shipper and commodity


These are the 11 things that MUST be on a driver's logs at all times. Both ELD and paper logs.
